The short answer
This charge is for your Medicare Part B (or other Medicare part) monthly premium, billed by the Centers for Medicare & Medicaid Services (CMS).
Medicare Premium Deductions via Easy Pay
CMS MEDICARE PREM on your bank statement is a monthly premium payment collected by the Centers for Medicare & Medicaid Services, the federal agency that runs the Medicare program. Most people see this as a Medicare Part B premium, which covers outpatient medical services, doctor visits, and preventive care. The standard Part B premium for 2024 was $174.70 per month, though your exact amount may differ if you pay an income-related adjustment surcharge known as IRMAA.
This charge appears when you are enrolled in Medicare Easy Pay, the program's automatic direct debit system. If you collect Social Security benefits, the premium is normally deducted from that monthly payment and never hits your separate bank account. You will see a CMS MEDICARE PREM bank debit only if you are not yet collecting Social Security, your benefit amount falls short of the premium, or you enrolled in Easy Pay as your preferred payment method.
If the amount looks wrong, log in at medicare.gov or call 1-800-MEDICARE (1-800-633-4227) to confirm what billing period the payment covers and whether an IRMAA adjustment was applied. Premiums change annually, which explains small year-over-year differences. If you believe the charge is an error or you never authorized Easy Pay, contact CMS before disputing with your bank — reversing a federal payment can create enrollment complications that take weeks to resolve.
